Chiropractic Outperforms Massage for Cervicogenic Headache Relief

Both massage and chiropractic care are popular treatments for headaches, and both can provide relief. But are they equally effective? A head-to-head comparison published in the journal Chiropractic & Osteopathy provides a clear answer for cervicogenic headache: spinal manipulation outperforms massage alone.

The Study Design

Researchers enrolled 80 patients with clinically diagnosed cervicogenic headache and randomized them to receive either high-velocity cervical spinal manipulation or low-load craniocervical mobilization (a light manual technique similar to massage). Both groups received treatment twice per week for six weeks.

Results

The spinal manipulation group demonstrated significantly greater reductions in headache frequency, intensity, and duration. They also reported better functional outcomes — improved ability to perform daily activities without headache interference. The benefits were present at the end of treatment and maintained at follow-up assessments.

Why Manipulation Is More Effective

Cervicogenic headache stems from specific mechanical dysfunction in the upper cervical facet joints. These joints become hypomobile — restricted in their normal range of motion — due to injury, chronic postural stress, or degeneration. High-velocity manipulation specifically targets this restriction, producing a neurological cascade that reduces pain signal output from the affected joints. Low-load techniques and massage address muscular tension but do not produce the same joint-level mechanical correction.
