What to Expect
Your First Appointment, Step by Step
1. Check-In & Intake
Arrive 15 minutes early to complete your new patient forms (or download them below). Bring any accident reports, insurance info, and your attorney's contact details.
2. Comprehensive Evaluation
Your DC will perform a full history review and physical examination, identifying all injury sites and establishing a baseline for your medical record.
3. Treatment Begins
In most cases, treatment begins on day one. Your DC will explain your diagnosis, your treatment plan, and how the lien process works.

How Lien-Based Care Works
We Sign a Lien Agreement
Before or at your first visit, we sign a medical lien agreement with you and your attorney. This legally assigns payment from your settlement to our clinic.
You Receive Care Immediately
Treatment begins right away — no waiting for insurance approval, no out-of-pocket payments. Your health comes first.
We Collect at Settlement
When your case settles, our fees are paid directly from the settlement proceeds. You never pay us directly.
